1.DOLE- Duly accomplished application form for AEP Application;
2.AEP Expiring Card (in Case of Renewal);
3. Photocopy of passport bio-page of Foreign National (FN) with latest admission and valid
visa,and appropriate visa in case of renewal;
4. Two (2) Original copy of Notarized Contract of Employment/Appointment or Board
Secretary's Certificate of Election enumerating the duties and responsibilities, annual salary
and other benefits of FN.
5. Six (6) pieces 2×2 1D picture with white background;
6. Photocopy of TIN ID with BIR 1904 or 1902 received by BIR;
7.Proof of Publication of the job vacancies in a newspaper of general circulation, together with
a duly notarized affidavit stating no applications were received orno Filipino applicant was
considered for the position;
8. Authorization letter from the company or foreign national and photocopy of their IDs;
9. Notarized Original Board Resolution;
10.Joint letter request addressed to the Commissioner from the applicant and the petitioner
which states:
(1)
that all documents are genuine;
(2) the specific Foreign Service Post (FSP) where said visa is to be forwarded;
(3) specific BI office to implement approved visa;
11. Duly accomplished CGAF for 9(g) visa issuance under IATF Resolution No. 131-A in two (2)
copies;
12. Certififed True copy of petitioner's latest Income Tax Return (ITR) with the corresponding
proof of payment (official receipt, bank teller's validation slip, BIR's eFPS payment details'print
out or other similar evidence);

13. a. For Corporations or Partnerships, CERTIFIED TRUE COPY of the following: i. Securities
and Exchange Commission (SEC) Certificate of Registration; ii. Articles of Incorporation; iii.
General Information Sheet (GIS) for the current year stamped received by the SEC; iv. Business
Permit with OR attached
14. Photocopy of Alien Employment Permit (AEP) issued by the Department of Labor and
Employment (DOLE); and actual publication of the applicant's approved AEP or in the absence
thereof, a Certificate of Publication issued by the Publisher;
